Social workers, case managers and discharge planners form an integrated team to ensure that patients -- who are medically stable -- and/or their families have the knowledge and resources necessary for a safe discharge. All social workers have a Masters degree and offer counseling and support to patients and families throughout the hospital stay. Additionally, they serve as liaisons between patients and long-term care and other alternative facilities, and assist in linking patients to appropriate state and federal programs when financial challenges arise.

Discharge planners are registered nurses who assess the progress of each patient, collaborate with attending physicians and other health care providers, and arrange for home care services and equipment as needed.

Case managers, also registered nurses, serve as a link between health care team members, patients, families and the Medical Center. JFK Medical Center has an on-site Medicaid worker who is available by appointment to assist patients and their families in the Medicaid application process. And, the Coordinated Health Department provides office space for a representative of the Visiting Nurses Association of Central New Jersey -- the only Medicare-certified home care agency in Middlesex County.
