JFK at Home is an expansion of the quality healthcare services available at the Medical Center with one special difference…the care is provided in the comfort and convenience of the patient’s home.  In partnership with Meridian Health System, patients can receive quality care in the comfort of their own home rather than at a hospital or long-term care facility. The program offers a breadth of health and home care services to meet the diverse needs of our patients. JFK at Home provides a coordinated system of comprehensive, multidisciplinary healthcare services in a patient’s home to promote, maintain and/or restore health, and minimize the effects of illness and disability. Upon the initial home visit, our skilled nurses — working closely with a team of healthcare professionals — assess the patient’s home environment, physical and clinical needs; as well as coordinate the patient’s plan of care with his/her physician to ensure the best outcome. Additionally, JFK at Home aides are available to make the patient’s recuperation at home a comfortable experience, performing a variety of in-home services including personal care, daily light housekeeping, and grocery shopping, among other tasks.
