Please be advised that effective May 1, 2019 Hackensack Meridian Health JFK Medical Center, Johnson Rehabilitation Institute and JFK Hartwyck Nursing and Rehabilitation Centers, and Empire HealthChoice HMO, Inc. and Empire HealthChoice Assurance, Inc. (Empire) will be ending their contractual relationship.

Hackensack Meridian Health still participates with a variety of insurance plans. Patients seeking information on accepted Hackensack Meridian Health insurances can visit www.hackensackmeridianhealth.org/insurance.

Effective May 1, 2019, Empire members may continue to use JFK Medical Center under the required wind down period. The wind down period for all hospitals is 60-days ending June 30, 2019.

Empire patients with out-of-network benefits may opt to receive care at non-participating hospitals. However, use of JFK Medical Center and other Hackensack Meridian Health facilities may trigger additional patient financial responsibility. It is also important to note that Empire members who do not have out-of-network benefits will always have access to emergency treatment at a Hackensack Meridian Health hospital.
