Talya Fleming, M.D.

Talya Fleming, MD is the Director of the Aftercare Program at JFK Johnson Rehabilitation Institute. She oversees the post-acute care treatment of patients while guiding them through the rehabilitation continuum. She has a special interest in neuro-stroke rehabilitation and has been integral in the development of the distinctive and well-sought after Stroke Recovery Program at JFK Johnson Rehabilitation Institute. Dr. Fleming attained admittance into the distinguished Phi Beta Kappa Honor Society while obtaining an undergraduate degree from Rutgers University. She earned her medical degree from UMDNJ-Robert Wood Johnson Medical School in Piscataway, New Jersey. Following a year-long Internal Medicine Internship at Morristown Medical Center, Dr. Fleming completed her residency training at JFK Johnson Rehabilitation Institute where she excelled as a Chief Resident during her final year. As an Attending Physiatrist and gaining further expertise in the management of neurologic, orthopedic and cardiopulmonary rehabilitation, she found herself uniquely able to help her patients while they were being treated in hospital acute care settings, skilled nursing facilities and outpatient settings. She currently maintains Board Certification in Physical Medicine & Rehabilitation and is a Clinical Assistant Professor at Rutgers Robert Wood Johnson University Medical School. Dr. Fleming has been invited to present at various organizations regarding program development and the integration of patients within the rehabilitation continuum to improve patient care. She is currently a Co-Principal Investigator for a clinical trial looking at medical and functional outcomes of patients with stroke.
