JFK at Home provides a coordinated system of comprehensive, multidisciplinary healthcare services in a patient’s home to promote, maintain and/or restore health, and minimize the effects of illness and disability. Upon the initial home visit, our skilled nurses — working closely with a team of healthcare professionals — assess the patient’s home environment, physical and clinical needs; as well as coordinate the patient’s plan of care with his/her physician to ensure the best outcome. Additionally, JFK at Home aides are available to make the patient’s recuperation at home a comfortable experience, performing a variety of in-home services including personal care, daily light housekeeping, and grocery shopping, among other tasks.
